要約 目的:アフリベルセプト2mg(IVA 2mg)から8mg(IVA 8mg)へ切り替えた新生血管型加齢黄斑変性(nAMD)症例の6か月の治療成績を検討した。
対象と方法:千葉大学医学部附属病院でIVA 2mgにてtreat-and-extend加療中のnAMD症例のうち,投与間隔が8週以下で,2024年9月〜2025年2月の間にIVA 8mgへ切り替えた連続40例44眼を対象とした。切り替え前(BL),切り替え投与後初回(Visit 1),および6か月後(6M)の最高矯正視力(BCVA),中心網膜厚(CRT),中心脈絡膜厚(CCT),投与間隔,および疾患活動性について後ろ向きに検討した。また,切り替え前後で2週以上の投与間隔延長が得られた割合を検討した。
結果:治療継続可能であった32例36眼を解析した。BCVA,CRT,CCTおよび疾患活動性は有意な変化なく維持された。平均投与間隔はBLの7.0±1.2週から6Mで9.1±1.4週へと有意に延長した(p<0.01)。56%の症例で2週以上の投与間隔延長が達成され,延長可能群では切り替え時のCRTがより薄く,疾患活動性を認める割合は低かった。
結論:IVA 2mgで8週以下の投与間隔を要したnAMD症例において,IVA 8mgへの切り替えにより視機能および解剖学的所見を維持しつつ,56%の症例で2週以上の投与間隔延長が可能であった。
Abstract Purpose:To evaluate the 6-month outcomes after switching from intravitreal aflibercept(IVA) 2 mg(IVA 2 mg) to 8 mg(IVA 8 mg) in patients with neovascular age-related macular degeneration(nAMD) who required ≤8-week treatment intervals under a treat-and-extend regimen.
Subjects and Methods:This retrospective study enrolled 44 eyes of 40 consecutive patients with nAMD receiving IVA 2 mg under a treat and extend regimen who were switched to IVA 8 mg at Chiba University Hospital between September 2024 and February 2025. Only patients with treatment intervals of ≤8 weeks at the time of switching were included. Best-corrected visual acuity, central retinal thickness(CRT), central choroidal thickness, treatment interval, and disease activity were assessed at baseline(BL), first visit after switch(Visit 1), and at 6 months(6M). The proportion of eyes with ≥2-week extensions between BL and 6M was also assessed.
Results:Thirty-six eyes of 32 patients completed the 6-month follow-up. Best-corrected visual acuity, CRT, and central choroidal thickness were maintained without significant changes throughout the study period. No significant difference in disease activity was observed at 6M. The mean treatment interval was significantly extended from 7.0±1.2 weeks at BL to 9.1±1.4 weeks at 6M(p<0.01);56% of the eyes achieved extensions of ≥2 weeks. The eyes with successful treatment extensions had thinner CRT and lower disease activity at the time of switching.
Conclusions:Switching to IVA 8 mg for eyes with nAMD requiring ≤8-week intervals under IVA 2 mg maintained functional and anatomical outcomes while enabling ≥2-week extensions in 56% of the patients.
