Abstract Purpose:Central serous chorioretinopathy(CSC) has been suggested to be associated with psychological stress;however, its relationship with chronic stress remains unclear. This study aimed to evaluate the association between CSC and chronic stress using hair cortisol concentration(HCC), a biomarker reflecting long-term endogenous cortisol exposure.

Methods:Nineteen patients with CSC and seven healthy controls who visited Osaka Metropolitan University Hospital between January and August 2025 were enrolled. CSC was diagnosed based on the presence of subretinal fluid(SRF) on optical coherence tomography and fluorescein leakage on angiography. Hair samples were collected from the occipital region, and HCC was quantified using an enzyme-linked immunosorbent assay(ELISA). HCC levels were compared between groups, and correlations with age, sex, smoking history, disease duration, and disease activity were analyzed within the CSC group.

Results:The mean HCC was 73.9 pg/mg in the CSC group and 49.8 pg/mg in controls, with no significant difference between groups(p=0.48). No significant correlations were found between HCC and age, sex, smoking history, or disease activity. However, patients with disease duration <6 months tended to show higher HCC values(mean 110.2 pg/mg) than those with duration ≥6 months(41.6 pg/mg).

Conclusions:Although overall HCC did not differ significantly between CSC patients and controls, elevated HCC in the early phase of disease suggests that transient increases in endogenous cortisol may contribute to CSC onset. Larger studies stratified by disease stage are warranted to clarify this association.
