要約　目的：網膜静脈分枝閉塞症に伴う黄斑浮腫（BRVO-ME）に対するファリシマブ硝子体内注射（IVF）の6か月治療成績を検討した。

方法：2024年4〜10月に大阪公立大学医学部附属病院でIVFを施行し，6か月以上経過観察可能であったBRVO-ME 31例31眼を対象とした。視力（logMAR値），中心窩網膜厚（CMT），中心窩脈絡膜厚（CCT），投与回数を治療前，3か月，6か月時点で評価し，新規治療群9眼と既治療群22眼で比較した。また治療レジメン別でpro re nata（PRN）群13眼（うち新規治療群6眼）とtreat-and-extend（TAE）群18眼（うち新規治療群3眼）でも比較を行った。

結果：新規治療群，既治療群の比較では，平均視力は両群とも有意差はなく（p＝0.07，0.23），CMTは新規治療群で有意に減少し（p＜0.05，0.07），CCTは既治療群で有意に減少していた（p＝0.37，＜0.05）。治療レジメン別の比較では，平均視力，CMTで両群とも有意差はなく（平均視力：p＝0.05，0.29，CMT：p＝0.29，0.11），CCTはTAE群に有意に減少を認めた（p＝0.05，＜0.05）。6か月間の平均投与回数は，新規治療群ではPRN 1.8回，TAE 4.7回，既治療群ではPRN 1.7回，TAE 4.2回であった。

結論：BRVO-MEに対するIVFは，6か月の短期観察で形態的改善と視機能改善傾向を示し，治療負担軽減に寄与する可能性が示された。

Abstract　Objective：To evaluate the therapeutic outcomes and potential to reduce treatment burden of intravitreal faricimab（IVF） for macular edema（ME） secondary to branch retinal vein occlusion（BRVO） at 6 months.

Methods：This retrospective study included 31 eyes from 31 patients with BRVO-ME who received IVF at Osaka Metropolitan University Hospital between April and October 2024, with at least 6 months of follow-up. Best-corrected visual acuity（BCVA, logMAR）, central macular thickness（CMT）, central choroidal thickness（CCT）, and injection numbers were assessed at baseline, 3 months, and 6 months. Subgroup analyses were performed between treatment-naïve and previously treated eyes, and between pro re nata（PRN） and treat-and-extend（TAE） regimens.

Results：CMT significantly decreased in the treatment-naïve group（p＜0.05）, while CCT significantly decreased in the previously treated group（p＜0.05）. BCVA showed a non-significant trend towards improvement in both groups. The PRN group showed a marginally significant improvement in BCVA（p＝0.05）. The mean number of injections was lower than that in fixed dosing regimens of pivotal trials.

Conclusion：Eyes treated with IVF for BRVO-ME demonstrated short-term functional and morphological improvement with fewer injections, suggesting IVF potential to reduce treatment burden in real-world clinical practice.
