Pit pattern diagnosis for colorectal lesion

Pit pattern分類は大腸病変の腺管開口部形態にもとづく分類で，大腸内視鏡診断のゴールド・スタンダードとして用いられている。Ⅰ型からⅤ型に分類され，質的診断から深達度を含む量的診断まで幅広く応用される。JNET分類が普及した現在も，JNET type2B病変を呈する病変や，確信度の低い病変に対する追加診断としてその価値は高い。一方，診断に不可欠なクリスタルバイオレットは発癌性の問題から使用が制限されつつあり，代替法の開発が進められている。

　The pit pattern classification is based on the shape of colorectal crypt openings and serves as the gold standard of colonoscopic diagnosis. Classified from Type I to V, it supports both qualitative and quantitative (invasion-depth) diagnosis. Even in the era of the JNET classification, it remains valuable as an additional tool for low-confidence lesions. However, crystal violet-essential for this diagnosis-now faces use restrictions due to carcinogenicity concerns, prompting the development of alternatives such as indigo carmine with TXI, acetic acid-enhanced NBI, and AI-based virtual chromoendoscopy.
