How to choose a suitable resection method for each lesion

要旨

大腸病変に対する内視鏡治療は，内視鏡技術や処置具の進歩により，多様な治療オプションが利用可能となった。現在ではpolypectomy/EMR，ESDだけでなく，cold polypectomyや浸水下手技などが広く行われている。ESDのようにほぼ適応が定まっている病変もあるが，それ以外の病変に関しては治療選択は術者に依存するところが大きい。また正確な術前診断が不可欠であり，病変の大きさ，肉眼型，悪性度を考慮した適切な手技の選択が重要である。本稿では，国内外の臨床研究やガイドラインをレビューし大腸治療内視鏡手技の適切な選択について考察する。

　Endoscopic treatment for colorectal lesions has evolved with advances in endoscopic technology and therapeutic devices, providing diverse treatment options. Currently, not only polypectomy/endoscopic mucosal resection (EMR) and endoscopic submucosal dissection (ESD), but also cold polypectomy and underwater techniques are widely performed. While certain lesions, such as those indicated for ESD, have well-established treatment algorithms, the selection of therapeutic approaches for other lesions largely depends on the endoscopist’s discretion. Accurate optical diagnosis is essential, and appropriate selection of techniques should consider size, morphology, and degree of malignancy. This review examines domestic and international clinical studies and guidelines to discuss the appropriate selection of endoscopic therapeutic techniques for colorectal lesions.
