要　旨

緊急内視鏡はそれ自体リスクが高い処置であるが，対象患者の状況によってはさらにリスクが高くなりうる。本稿では患者の対象や状況によるリスクを整理することにより，できるだけリスクを減らすことができるような「心得」となる内容をまとめた。緊急内視鏡は治療のメインであるが，リスクを回避でき治療が成功するかどうかの分かれ道は内視鏡を始める前の準備段階から始まっている。リスク管理の鉄則は「大きく構えて小さく収める」ことであり，時には「勇気ある撤退」も辞さないことが重要である。

　Emergency endoscopy is a high-risk procedure in itself, but it may be an even higher-risk procedure depending on the situation of the target patient. In this section, we have summarized the contents that will be “knowledge” that can reduce the risk as much as possible by organizing the risk depending on the patient’s target and situation. Emergency endoscopy is the main treatment, but risk avoidance and successful treatment begin in the preparatory stage before starting endoscopy. The ironclad rule of risk management is to “hold big and keep small”, and sometimes it is important to keep “courageous withdrawal”.
