要旨

　32歳男性。身長188cm，体重175kg（BMI：49.5kg・m^－2）。磁気共鳴画像（MRI）検査で右前頭葉腫瘍を指摘され覚醒下手術予定となった。高度肥満のため術中呼吸管理のリスクが高いことから鎮静薬を使用せず，片側の頭皮神経ブロックと高流量鼻カニューレ（HFNC）を用いAwake-Awake-Awake法で麻酔管理を行う方針とした。術中は疼痛コントロールも良好で問題なく経過し覚醒下手術を施行可能であった。

　The patient was a 32-year-old man（height：188cm, weight：175kg, BMI：49.5kg・m^－2）. MRI had revealed a right frontal lobe tumor, and awake surgery was scheduled for the tumor’s resection. Due to the patient’s severe obesity, there was a high risk of respiratory issues during an awake craniotomy. We thus elected to not use sedatives and to manage the patient’s anesthesia using the Awake-Awake-Awake method, with a unilateral scalp block and a high-flow nasal cannula. The intraoperative pain management was sufficient, and the surgery was completed without problems.
