要旨
体外循環補助が困難な右総腸骨動脈解離と腹部大動脈瘤合併低心機能(左室駆出率22%,肺うっ血合併)重症大動脈弁狭窄症に対し局所麻酔・鎮静下(プロポフォール,ケタミン,デクスメデトミジン)に大動脈内バルーンパンピング(intra-aortic balloon pumping:IABP)を用いて経カテーテル的大動脈弁植え込み術(transcatheter aortic valve implantation:TAVI)を行った。IABPは比較的簡便かつ低侵襲であり,低心機能で術中の循環動態の破綻が懸念されるTAVIにおいて有効な補助手段である。
In transcatheter aortic valve implantation(TAVI)for aortic stenosis, there are cases in which low cardiac function prevents maintenance of circulatory function during the procedure, especially after rapid pacing. In such cases, there are reports of TAVI using veno-arterial extracorporeal membrane artificial lung(V-A ECMO), but few reports of TAVI with intra-aortic balloon pumping(IABP). In this report, we describe a case in which TAVI was performed safely and effectively with IABP in a patient with severe aortic stenosis and low cardiac function.
