 Double-eyelid surgery has long been one of the most commonly performed aesthetic operations in Japan. The non-incisional method, known for its short recovery and minimal invasiveness, is typically chosen for first-time patients. In contrast, the incisional approach offers firmer and longer-lasting fixation because the surgeon can work directly under visual control. In recent years, the differences between these two procedures have become less distinct. Advances in suture placement have improved the longevity of the non-incisional technique, and delicate microsurgical closure has made the incisional blepharoplasty appear more natural and less ʻscary.ʼ Choosing the right method requires not only a precise anatomical assessment but also an understanding of each patientʼs expectations and daily life. Over time, the non-incisional technique may lead to thread exposure, granuloma, and/or fading of the crease, whereas the incisional approach can result in scarring, upper-eyelid hollowing, and/or sensory change. Modern eyelid surgery now focuses on harmony — among the eyelid crease, brow position, periorbital volume, and facial expression — rather than the crease alone. For lasting and balanced outcomes, a flexible and individualized plan is essential.
