 The efficacy of transconjunctival Müller muscle tacking surgery alone for bilateral blepharoptosis is described. In the field of plastic surgery in Japan, most ptosis surgeries are performed by advancement of the levator tendon from the cutaneous side, but in some cases the operation time is longer and the postoperative downtime is more noticeable because the width of the left and right eyelid opening is adjusted. However, in 20 cases, we approached from the conjunctival side and tucked only the Mullerʼs muscle directly (kamei method), resulting in almost no noticeable swelling immediately after surgery, a shorter operation time, and symmetry without the need to adjust the amount of eyelid lift. In addition, the double eyelid was naturally formed without any modification, and the concavity of the upper eyelid was also improved. This suggests that acquired ptosis is not caused by a problem with the eyelid elevator muscle, but by an abnormality in the function of the Müller muscle.
