 A 66-year-old female presented with a rash on the left side of her labia majora that had first appeared 3 years earlier. The rash was actually a 30×4-mm linear black lesion that was diagnosed as superficial basal cell carcinoma (BCC) by excisional biopsy. With the patient under local anesthesia, we excised the BCC with 5-mm surgical margins and a partial fat layer. There has been no recurrence at 1 year post-surgery. BCC usually occurs on the face of elderly individuals; the development of BCC on the genitalia is rare. Among the various BCC clinical patterns, linear BCC is an uncommon morphologic variant. It is defined as ʻa lesion that extends preferentially in one direction, resulting in a tumor or plaque with a length much greater than its width greater than 3:1 ratioʼ. There are many unclear points about the oncological character of linear BCC, and clinicians should thus become familiar with this BCC variant.
