 Plastic surgeons play a vital role in the treatment of gender incongruence by addressing gender dysphoria through the implementation of procedures that align physical characteristics with a patientʼs gender identity. Among these procedures, chest masculinization surgery, including mastectomy for transmasculine individuals assigned female at birth (AFAB), is one of the most frequently performed operations. Increasingly, the focus has shifted from simply removing breast tissue to achieving a natural male chest contour, spurring global interest in the definition of male chest aesthetics. The nipple-areolar complex (NAC) in particular has garnered attention for its critical role in achieving a natural appearance, with studies examining the optimal position and morphology of the NAC on the chest wall.

 Despite various methods for measuring NAC position and morphology, the quantitative standardization remains limited. However, consensus does exist regarding the approximate placement and shape of the NAC, emphasizing the need to consider patientsʼ thoracic and torso proportions. Further refinement of surgical techniques is essential to achieve more natural and consistent outcomes. Successful surgery demands not only technical expertise but also collaboration with patients to establish a shared understanding of the patientsʼ aesthetic goals.
