 Periorbital deformities that occur after maxillary reconstruction include enophthalmos, lower-lid ectropion, lower-lid retraction, and orbital-rim depression. Possible mechanisms of such deformities include increased intra-orbital volume, lower-lid scar contracture, drooping of transplanted tissue, orbicularis oculi paralysis, and periorbital bone defects. Adjuvant radiotherapy before and after surgery also significantly affects eye and eyelid deformities. The degree of deformity varies from case to case and must be assessed by visual inspection, palpation, and imaging findings; corrective procedures should be considered. The most important consideration is to minimize the deformity after the initial operation, and we believe that rigid support of the orbital contents and periorbital bone reconstruction based on the buttress theory are important for this purpose.
