 The numerous facial muscles that create facial expressions include the orbicularis oculi, orbicularis oris, and zygomatic major, but a major issue presented by burns is that the skin surface becomes hard due to scarring, making it difficult to form soft expressions. Particularly in cases of extensive burns, saving lives is prioritized, making it challenging to provide adequate initial treatment that considers the cosmetic and functional aspects of the face. For the secondary reconstruction and touch-up surgery of facial burns, it is crucial to clarify how hypertrophic scars and scar contractures occur by assessing their causes and risks. It is necessary to understand the tension on the skin and systemic factors known as keloid predisposition. Reconstruction is preferable after the inflammation has peaked, and the choice between using grafts or flaps must be considered. Since no two patients have the same scars, it is important to carefully consider the timing and method of reconstruction for each case and approach treatment accordingly.
