塞栓硬化療法が有効であった耳介動静脈奇形の1例

An Auricular Arteriovenous Malformation Successfully Treated with Embolosclerotherapy

はじめに

　耳介に存在する動静脈奇形（arteriovenous malformation：以下，AVM）では，潰瘍や出血を来たした場合には治療の絶対適応と考えられる。この際，耳介であるがゆえに整容面への配慮も重要であり，手術による治療では瘢痕や変形などが問題となることが多い。一方，手術を併用しない塞栓療法や塞栓硬化療法では，皮膚壊死の合併症が見られない場合には整容面での新たな問題が生じにくい。今回われわれは，塞栓硬化療法のみで耳介AVMが著明に改善した症例を経験したので報告する。

　We treated a 50-years-old female with an auricular arteriovenous malformation (AVM) that markedly improved with embolosclerotherapy alone. Although auricular AVMs may require amputation of the entire auricle, it is important to consider the cosmetic aspects of the auricle during treatment. We followed the previously reported treatment algorithm for AVMs and prioritized embolosclerotherapy combined with coil embolization and sclerotherapy when varicose vein dilation was observed immediately after AV shunting.
