Key Points of the Physical Examination for the Diagnosis of Lymphedema

はじめに

　リンパ浮腫とは，蛋白質などを含む体液が，何らかの原因でリンパ管に回収されずに組織間に貯留したものであり，原因となる病態は多岐にわたる。近年，疾患の認識の広まりとともに「むくみ」を主訴として受診する患者が増加しており，迅速かつ適切に診断を得るためには，丁寧な問診と正確な理学所見が重要となる。理学所見は，診察する側の技量や主観がある程度影響するため，明確なエビデンスを構築するのは困難だが，習熟すればコメディカルでも治療に直結する有益な情報を得ることができる。本稿では，リンパ浮腫の概要と，リンパ浮腫が疑われる患者を念頭に聴取すべき病歴や理学所見のポイントについて解説する。

　To diagnose lymphedema of the extremities, it is essential to obtain an appropriate medical history and perform physical examinations related to the edema. Physical findings of this lymphedema would include changes in skin color, pitting or non-pitting edema, skin thickeness, lymphorrhea, and redness with cellulitis. Limb circumference measurements are the most commonly used routine in-office assessments and should be repeated regularly.

　Although advanced diagnostic imaging technology provides various types of information including the presence of lymphatic vessels and the quality of their walls, a careful and accurate physical examination is crucial to establish a prompt diagnosis and gather valuable information for the patientʼs care.
