 Lymphatic imaging using indocyanine green(ICG), which originated in Japan, has two properties that are suitable for subcutaneous lymphatic imaging:(1)the excitation and emission of ICG penetrates living tissue easily, and(2)ICG injected subcutaneously will be absorbed into the lymphatic system easily and selectively. In general, the lymphatic system can be visualized to a depth of 2 cm. The upper extremities can be visualized up to the axilla, and the lower extremities can be visualized well below the knee.

 Although lymphatic imaging using ICG is a convenient examination, the technique and the interpretation of the results are highly flexible, and the results can vary greatly depending on the skill of the examiner. In this article, we present some aspects of ICG fluorescence lymphography that are necessary for the diagnosis and surgical approach. We also demonstrate functional ICG fluorescent lymphography based on a new functional lymphatic anatomy.
