長期経過で明瞭化した膵管内管状乳頭状腫瘍（ITPN）の1例

A case of intraductal tubulopapillary neoplasm（ITPN）of the pancreas with gradual radiologic clarification over a long-term course

intraductal tubulopapillary neoplasm（ITPN）は，粘液産生の乏しい高度異型上皮によって構成される膵管内腫瘍である。今回我々は，右卵巣癌術後の経過観察中に指摘され，ITPNと診断された症例を経験した。後方視的にみると7年前からの単純CTの経過で主膵管の軽度拡張が出現，経過でいったん明瞭化，その後は不明瞭化していたが，今回再度明瞭化していた。主膵管内を充満するように緩徐に増大した病変であることが示唆された。そこで本症例について若干の文献的考察を加えて報告する。

Intraductal tubulopapillary neoplasm（ITPN）is a rare intraductal pancreatic tumor characterized by highly atypical epithelial cells with minimal or absent mucin production. We present a case of ITPN that was incidentally identified during routine postoperative surveillance following surgery for right ovarian cancer. On retrospective review of non-contrast CT images obtained over a 7-year period, mild dilatation of the main pancreatic duct was initially observed, became more conspicuous during follow-up, and subsequently became less apparent. These imaging findings suggested a slowly growing intraductal lesion that progressively occupied the lumen of the main pancreatic duct.
