被膜破綻による広範囲な出血をきたした漿液性境界悪性卵巣腫瘍茎捻転の1例

A case of serous borderline ovarian tumor torsion with rupture resulting in hemoperitoneum

卵巣腫瘍茎捻転は，卵巣あるいは卵管が固有卵巣索や卵巣提索の支持靱帯を軸にねじれることにより卵巣に血行障害をきたす疾患である^1）。あらゆる年齢の女性に起こり得るが，症状が非特異的で鑑別診断が多岐にわたるため，画像による術前診断が重要な疾患である。癒着のない良性卵巣腫瘍に多く発生することが知られており^2），悪性卵巣腫瘍に発生することはまれとされ^3），境界悪性卵巣腫瘍における茎捻転の報告はほとんどない^4）。また，茎捻転で生じる出血は通常腫瘍内部や腫瘍壁に生じることが多く^5）6），腫瘍周囲に出血が広がることはまれである。今回我々は，被膜破綻により腫瘍周囲に広範囲な出血をきたした漿液性境界悪性卵巣腫瘍茎捻転の1例を経験したので報告する。

Torsion of ovarian tumors typically occurs in benign neoplasms, whereas it is rare in malignant or borderline tumors due to reduced mobility caused by adhesions or invasion. Moreover, rupture associated with torsion is uncommon. We report a rare case of torsion of a serous borderline ovarian tumor accompanied by rupture, resulting in massive hemorrhage around the tumor. Preoperative evaluation of characteristic imaging findings of ovarian torsion and rupture, as well as the possibility of malignancy, is important.
