造影CTでの形態的特徴が鑑別診断に有用であった小腸神経内分泌腫瘍の1例

A case of small intestinal neuroendocrine tumor with diagnostic utility of contrast-enhanced CT morphology

神経内分泌腫瘍（neuroendocrine tumor：NET）は消化器（膵，直腸，胃，十二指腸，虫垂，空腸/回腸），肺，気管支など様々な臓器に発生する腫瘍である。粘膜深層の内分泌細胞から発生し，膨張性に発育することから，粘膜下を主座とし，肉眼的には粘膜下腫瘍の形態を呈することが多い。小腸NETは比較的まれであり，画像診断は困難なことが多いが，造影CTによる形態的診断の有用性については，十分に認識されていないのが現状である。今回我々は，造影CTでの形態的特徴が鑑別診断に有用であった小腸NETの1例を経験したので，若干の文献的考察を加え報告する。

A 67-year-old woman was found to have an incidental small bowel mass during a CT scan for postoperative follow-up of endometrial cancer. Capsule endoscopy and double-balloon endoscopy suggested a submucosal tumor, and a biopsy was performed. Pathological findings raised suspicion of a neuroendocrine tumor, and a partial small bowel resection was carried out. Small bowel neuroendocrine tumors are rare, and both capsule endoscopy and CT findings are useful for differential diagnosis.
