Roux-en-Y再建後の急性輸入脚症候群の手術例に関するCT所見の検討

CT findings of surgical cases of afferent loop obstruction after Roux-en-Y reconstruction

急性輸入脚症候群はBillroth Ⅱ法やRoux-en-Y法（以下R-Y法）による再建後に発症するまれな合併症の一つで，輸入脚閉塞による内圧亢進に伴って，腸管の穿孔や壊死を伴い急激な病態悪化を呈することがある。今回我々は，R-Y再建後に急性輸入脚症候群となり，緊急手術を要した8例のCT所見などについて検討を行ったので，文献的考察を加えて報告する。

Afferent loop obstruction（ALO）is uncommon complication that occurs after Billroth Ⅱ and Roux-en-Y reconstruction. We reviewed CT findings of 8 cases of ALO requiring emergency surgery, which had revealed 4 cases of internal hernia and 4 cases of adhesion. At CT diagnosis of ALO caused by internal hernia, hooking intestine sign, mesenteric fat haziness, and vending, crowding mesenteric vessels and converging mesenteric fat by the side of stenosis were useful.
