外耳孔から突出した髄膜腫の1例

A case of meningioma protruding into the external auditory canal

髄膜腫の多くは硬膜に接する隆起性腫瘤の形態で知られているが，まれに硬膜に沿ってシート状・板状に発育する形態を呈する病態を示すことがあり，en plaque meningiomaと呼ばれている^1）。髄膜腫全体の発生部位としては中頭蓋窩が79.5％と最も多く，特に蝶形骨縁に多いとの報告があるが^2），外耳道に進展した髄膜腫症例は比較的まれである。今回我々は，20年の歳月を経て外耳孔から突出した髄膜腫（en plaque meningioma）の1例を経験したため，文献的考察を加え報告する。

We report a case of meningioma protruding into the external auditory canal in her 80s. CT and MRI showed strong and uniform contrast effect. MRI showed homogenous signal intensity and no diffusion limitation. The tumor had osseus invasion and combined with skull base dura mater thickening. Tumor biopsy was performed after arterial embolization to prevent hemorrhage. The diagnosis was meningioma. External auditory canal meningiomas are relatively rare, but the uniform tumor signal and enhancement, dural thickening and bone infiltration on CT and MRI may be the clues for collect diagnosis.
