転移巣の脂肪成分の存在が診断の契機となった低分化肝細胞癌膵転移の1例

A case of pancreatic metastasis of poorly differentiated hepatocellular carcinoma in which the presence of a fatty component in the metastatic lesion triggered the diagnosis

肝細胞癌は肝内に転移しやすく，肝外の転移先として主なものは，肺（55％），リンパ節（53％），筋骨格系（28％），副腎（11％）が知られているが，膵臓への転移は約1％と報告されており，非常にまれである^1）。また，悪性腫瘍全体の転移先としても，膵臓は全体の2～5％ほどと非常に少ない^2）。一方，転移性膵腫瘍の原発として主なものは，腎癌（30％），肺癌（23％），乳癌（12％），軟部肉腫（8％），大腸癌（6％），悪性黒色腫（6％）とされている^3）。今回，肝細胞癌の膵転移を生検前に画像的に診断し得た症例を経験したため報告する。

Pancreatic metastasis of hepatocellular carcinoma is very rare. In this case, contrast-enhanced CT showed a well-defined hypovascular pancreatic mass along the main pancreatic duct like tumor thrombus. The pancreatic mass demonstrated signal intensity loss on chemical shift imaging, suggesting fat content. Fat-containing and the shape like tumor thrombus were the key to diagnose pancreatic mass as metastasis of hepatocellular carcinoma.
