低悪性度子宮内膜間質肉腫のMRI像の把握，および臨床的問題点との関連性の考察

Morphological variations of low-grade endometrial stromal sarcoma on magnetic resonance imaging, and their relationship with clinical issues

低悪性度子宮内膜間質肉腫は，進行は緩徐ではあるものの，リンパ節転移や肺転移・腹膜播種をきたしうる，希少な悪性腫瘍である。MRIの形態として，子宮筋層の間を分け入るように芋虫状に進展する腫瘍形態が典型像としてよく知られる^1）。一方で，それとはまったく異なった，rim状のT2強調像低信号に取り囲まれ，境界明瞭に見える腫瘍形態も報告されている^2）。しかし，その希少性もあり，低悪性度子宮内膜間質肉腫の画像診断の報告は，いずれも数例での報告にとどまっており，多くの症例に基づいてのMRIの典型像およびその頻度，その他のとりうる形態，などの全体像の理解が難しい。

This multi-center retrospective study aimed to classify low-grade endometrial stromal sarcoma（LGESS）based on MRI characteristics and to explore their specific clinical pitfalls. Twenty-five cases of LGESS at seven institutions were classified into the following four subtypes based on MRI phenotypes：eight adenomyosis-like cases, 10 leiomyoma-like cases, one protruding case, and six sarcomatous cases. Adenomyosis-like and leiomyoma-like tumors tended to be preoperatively presumed benign disease, and leiomyoma-like tumor had high risk of inappropriate performance of myomectomy.
