This chapter introduces advanced technology related to boron neutron capture therapy(BNCT). Clinical trials for BNCT had been conducted using research reactors in the world in the past because the therapy needs high-intensity neutrons. Recent progress in technologies in accelerators and neutron sources with accelerators has realized “accelerator-based BNCT” which conducts the treatment by using the neutron beam generated by a compact accelerator. In the development related to accelerator-based BNCT devices, high-current compact accelerators and neutron-generating target materials in particular have been performed concentrated in recent years. And technologies controlling neutron energy and distribution such as beam-shaping assembly, collimators, and bolus were also sophisticated from the reactor-based BNCT devices. Based on a lot of developments and efforts, in the 2020 s some devices have succeeded to generate enough neutrons for the treatment. Peripheral devices relating to dose estimation, patient alignment technique, radiation measurements, etc. currently have several issues. Improvement of the equipment and software will contribute to establishing BNCT in the near future.
