ホウ素中性子補足療法（BNCT）の現状：臨床現場から

Current status of boron neutron capture therapy（BNCT）：from clinical practice

ホウ素中性子捕捉療法（boron neutron capture therapy：BNCT）は，腫瘍にホウ素薬剤（¹⁰B）を取り込ませたうえで熱中性子を照射する治療法である。これによりホウ素を取り込んだ細胞内でホウ素中性子捕獲反応［¹⁰B（n,α）⁷Li］を生じさせて，その結果生じる高linear energy transfer（LET）粒子線により腫瘍を破壊することができる。ホウ素中性子捕獲反応によって生じる高LET粒子線はその飛程が10μm未満と細胞の直径に満たないことから，ホウ素薬剤を取り込んだ細胞を選択的に破壊して，隣接する細胞のダメージを最小限にできることが大きな特徴である。腫瘍に選択的にホウ素薬剤を取り込ませることができれば，たとえ浸潤性に広がる腫瘍であっても周囲の正常組織のダメージは最小限にとどめながら腫瘍細胞だけを破壊することが可能になるという治療法であり，細胞レベルでの粒子線治療である^1）（図1）。

Boron neutron capture therapy（BNCT）is a cellular-scale particle therapy in which boron compounds are incorporated into tumor cells and then irradiated with thermal neutron. In this article, we have overviewed the history of BNCT and today’s clinical practice for head and neck cancer. The challenges for the future of BNCT and the involvement of radiation oncologists have then been discussed.
