長期経過で緩徐に増大した肺静脈瘤の1例

A rare case of pulmonary varix with gradually enlargement

肺静脈瘤は肺静脈が肺動脈との連続がなく，嚢状や蛇行状に拡張する病態である。現在まで95例しか報告されておらず非常にまれである。呼吸困難感，喀血，胸痛などの症状もあるが無症候性のものも多く，無治療のまま経過観察を終了することもある。今回，他疾患に合併し偶発的に発見された肺静脈瘤を経験し，長期の経過を観察できたので報告する。

An 81-year-old female underwent partial resection of Minimally Invasive Adenocarcinoma in left upper lung. Preoperative contrast-enhanced CT scan images showed a hyperdense small nodule in the right pulmonary hilar region, that was thought to be a small lymph node. Six years later, the nodule enlarged in follow-up simple CT scan images. Contrast-enhanced CT images revealed same contrast enhancement as pulmonary vein. A 3D reconstructed image showed it protruded from branch V2 of the right upper pulmonary vein without continuous pulmonary artery and was diagnosed as a pulmonary varix.
