脳腫瘍の診療における脳機能画像の現状と将来展望

Functional imaging for brain tumors：now and future

脳腫瘍の診療において，神経脱落症状は患者のQOL低下に直結するので，正常な脳神経組織の領域と腫瘍の領域を正確に診断することが重要である。悪性脳腫瘍などでは正常組織と腫瘍との境界がわかりにくいことも少なくないため，理学的検査や神経学的検査，一般的な画像検査のほかにも機能画像を含めた様々な検査を組み合わせて診断する必要がある。本稿では，脳腫瘍の代謝機能や状態を反映した画像検査として，FDG-PET，メチオニンPET，人工アミノ酸PET，MRS/MRSIを取り上げる。また，正常な脳神経機能を反映した画像検査として，fMRI，MR tractography，FDG-PETを取り上げ，それぞれの検査について概説する。

In the treatment of brain tumors, it is important to accurately diagnose the region of normal brain nerve tissue and the region of tumor because withdrawal symptoms are directly linked to the deterioration of the patient’s quality of life. In malignant brain tumors, it is often difficult to distinguish the boundary between normal tissue and tumor, and it is necessary to make a diagnosis by combining various examinations including functional imaging. In this paper, we will discuss FDG-PET, methionine PET, artificial amino acid PET, and MRS/MRSI as imaging exams that reflect the function or biological status of brain tumors. In addition, fMRI, MR tractography, and FDG-PET are taken up as imaging exams reflecting normal central nerve function.
