アルツハイマー型認知症およびレビー小体型認知症診断における脳機能画像の現状と将来展望

Current situation and future prospects of functional brain imaging in the diagnosis of dementia

高齢化社会が進むにつれアルツハイマー型認知症をはじめとした認知症の罹患者が今後ますます増えてくるものと危惧されている。レビー小体型認知症も認知症に占める割合が20％程度ともいわれており，アルツハイマー型認知症に次ぐ重要な変性疾患であるが，その病態に関してはまだよくわかっていない部分も多く，より正確な予後予測や治療方針決定のためには，これら認知症に対する新たな機能画像やバイオマーカーによる診断体系の確立が必要不可欠と思われる。

Alzheimer’s disease and dementia with Lewy bodies are becoming more and more prevalent as the aging population become advanced. Accurate diagnosis is indispensable for determining treatment strategies, and with the development of amyloid PET and tau PET, the imaging biomarker using nuclear medicine has become an important tool for dementia diagnosis. In this article, we review the conventional diagnosis of dementia using cerebral blood flow SPECT, DAT SPECT, and MIBG scintigraphy, and discuss the clinical significance of amyloid PET and tau PET, which are expected to be used in the future.
