甲状腺乳頭癌では，様々な部位への転移が認められることがある。転移のうち，30～80％の頻度でリンパ行性転移が認められる^1）。血行性転移の頻度は少ないが，骨，肺，皮膚，脳への転移が多いことが報告されている^2）3）。下咽頭への転移は非常にまれで，その報告は少ない。今回我々は，甲状腺乳頭癌術後15年目に認められた甲状腺乳頭癌の下咽頭転移の1例を経験したので，文献的考察とともに報告する。

We report a case of hypopharyngeal metastasis of papillary thyroid cancer detected 15 years after subtotal thyroidectomy. CT and MRI showed an enhancing submucosal mass in the hypopharynx with restricted diffusion. The mass was growing slowly. Metastasis of papillary thyroid carcinoma to the hypopharynx is very rare. In all previous case reports, the masses were submucosal or subepithelial. If a submucosal hypopharyngeal mass has a history of papillary thyroid carcinoma, metastasis of papillary thyroid carcinoma should be considered as a differential diagnosis.
