成人Wilms腫瘍の1例

A case of adult Wilms tumor

Wilms腫瘍は小児腎腫瘍において最も多い腫瘍である^1）が，成人における発症はまれである^2）。成人Wilms腫瘍の治療は小児のWilms腫瘍に準じて行われている^3）が，標準的な治療法は確立されていない。今回，我々は成人Wilms腫瘍の1例を経験したので，多少の文献的考察を交えて報告する。

Adult Wilms tumor is very rare. Therefore, treatment policy is not still settled. Here, we report a case of adult Wilms tumor who were successfully treated by following to the treatment protocol of child’s Wilms Tumor. The patient was 30s male with lung masses and a huge left renal mass which were pathologically diagnosed as Wilms tumor（Epithelial Type）by left nephrectomy. Chemoradiotherapy to renal bed with renal hilar lymph node metastasis（21.6 Gy/12 Fr）was performed as an adjuvant therapy. Besides, irradiation to bilateral lungs（12 Gy/8 Fr）and to liver（10.5 Gy/7 Fr）was added as lung metastases were progressed and liver metastases were newly appeared at four and six months later. Nine months later, second chemotherapy is being performed and he keeps no evidence of recurrence.
