近年，前立腺癌の病態に対する理解の深まりと画像診断技術の進歩および新規ホルモン薬を含めた治療選択肢の増加により，進行前立腺癌に対する治療は，大きな変化を遂げている。これまでは，転移性前立腺癌に対しては，①アンドロゲン除去療法単独，または，②抗アンドロゲン薬との併用療法，が均一に行われてきた。しかしながら最近では，③腫瘍量や転移部位に応じて治療法を変える，層別化が行われるようになった。これを行うために「病変の広がりと活動性を評価する画像診断法の確立」が重要な課題となっている。

Loco-regional therapy against oligo-metastatic cancer has generated great interest in prostate cancer. To apply loco-regional therapeutic strategy against castration-resistant prostate cancer（CRPC）, it is essential to accurately identify the progressive sites as the treatment targets. Diffusion-weighted whole-body imaging with background body signal suppression（DWIBS）is a promising imaging technique for CRPC as a single-step examination to assess osseous and extra-osseous involvement. It provides a clear categorization of the therapeutic response at each metastatic site, and enables identification of a progressive lesion. Loco-regional radiotherapy can be an effective treatment option for oligo-progressive CRPC.
