画像診断の教科書では，脳腫瘍に多くのページが割かれているが，脳腫瘍の発生頻度は国内で年間13,000～14,000人程度と推定され，多部位の癌と比べると希少である。また，脳卒中などのほかの脳疾患と比べても発生頻度は低い。一方，WHO分類における脳腫瘍の病理組織型は130以上と多く^1），臨床経過も画像所見も多彩である。したがって，画像診断においては脳腫瘍の多彩な画像所見を念頭に置きつつ，腫瘍以外の疾患との鑑別を考える必要がある。

本稿では，類似した画像所見を示す脳腫瘍および非腫瘍性疾患を並べつつ呈示し，それぞれの画像所見と鑑別のポイントを解説する。

In the central nervous system, the differentiation between neoplastic and non-neoplastic disorders is occasionally difficult. In this paper, I will present several neoplastic and non-neoplastic cases which show similar imaging findings and explain the main points of differential diagnosis.
