Hemosuccus pancreaticusの1例―造影CTで仮性動脈瘤からのextravasation

Hemosuccus pancreaticus with CT demonstration of extravasation from pseudoaneurysm and successfully controlled by TAE：a case report

hemosuccus pancreaticusは，膵管を経由したVater乳頭からの消化管出血で，胆道出血と区別するためにSandblomにより1970年に提唱された^1）。原因として慢性膵炎を基礎に形成された膵仮性嚢胞や仮性動脈瘤からの出血が多い。重篤な病態ではあるが，間歇的な出血のため，しばしば上部内視鏡検査で十二指腸乳頭からの出血が認められない。そのような症例に造影CTを施行したところ，脾仮性動脈瘤からの造影剤の血管外漏出が主膵管を経由して十二指腸に注ぐのが描出された，世界初と思われるhemosuccus pancreaticusの症例を経験したので報告する。

Hemosuccus pancreaticus is a rare cause of gastrointestinal hemorrhage from the ampulla Vater via pancreatic duct. We report a case of hemosuccus pancreaticus with CT demonstration of extravasation of contrast media from the ruptured splenic pseudoaneurysm into the pancreatic duct and with successful hemostasis by TAE. To our knowledge, this is the first case of direct CT demonstration of bleeding from the papilla Vater through pancreatic duct.
