は じ め に
末期変形性足関節症（足OA）に対する手術方法として足関節固定術（ankle arthrodesis：AA）と人工足関節全置換術（total ankle arthroplasty：TAA）が選択される．AAの目的は良好な距腿関節のアライメントと骨癒合の獲得である．固定方法や軟部組織に対する処置方法の改善により合併症は減少し，骨癒合率や除痛効果は向上してきた．TAAの目的は障害されている関節を人工関節に置換することで，足関節の機能を温存することである．開発当初の1970年代と比較して，インプラントデザインの改善などにより，生存率や患者満足度，除痛効果，可動域などの臨床成績は向上してきた．
本稿ではAAとTAAに対する特徴や比較について説明する．
