胸腔内の癒着は，術前に画像所見や病歴，職歴などからある程度が予想可能とはいえ，予期せず強固な癒着症例に遭遇することも少なくない．その際の剝離操作の巧拙は出血量や手術時間，肺損傷などの手術侵襲に直接かかわり，ひいては術後合併症の発生の多寡にも影響をもたらす．よって常日ごろから呼吸器外科医たるものは，明日遭遇するかもしれない癒着症例に対して適切な対応ができるようになっていなければならない．症例ごとに癒着の程度や範囲が異なるとはいえ，癒着剝離にはいくつかの共通するコツやセオリーが存在する．ここでは特に全面癒着時の剝離手順を中心に述べ，今後の参考としていただけたらと考えている．
