【Purpose】This study aimed to clarify the current status of pregnant women with psychiatric disorders and to examine the care required of midwives in early pregnancy.

【Method】The research period was from July 2017 to December 2017. The subjects were first-time mothers of less than 14 weeks with a score of 12 or more on the EPDS or a "yes" response to the Whooley's 2-item questionnaire, who had a history of or complications of mental illness in early pregnancy. A semi-structured interview method was used for data collection. The content analysis method of Berelson, B was used for the analysis. Reliability was checked using Scott's equation. This study was approved by the Ethics Committee (No. 2926).

【Results】The subjects were four patients with a history of psychiatric disorders, and their mean age was 31.3 years. As a result of the data collection and analysis, 324 numbers of units of record, 53 subcadegories, 16 categories, and 5 core categories were extracted. The core categories extracted were: 1. "Perceptions of the patient and family about the pregnancy," 2. "Physical symptoms," 3. "Acceptance of mental illness and thoughts," 4. "Anticipated anxieties and expectations" and 5. "Desires for midwives".

【Conclusions】

1. Patients are unable to judge or control their own physical symptoms.

2. Her perception of pregnancy was that she was worried about her genetic predisposition, which affected her mental state.

3. Anxiety and expectation of family support system for pregnancy caused stress and hard feelings, and they sought support from their own mother.

4. Lack of knowledge about genetic information and the effects of drugs and the relationship with the specialist caused some pregnant women to discontinue treatment due to inability to communicate well with the doctor.

5. Awareness of midwives is low, and they do not know the specifics of midwifery work.
