  The purpose of this study was to identify the appropriateness of nursing outcomes and nursing interventions for NANDA Nursing Diagnosis “Risk for Infection”in nursing care plans of nursing records among 11 hospitals in Japan. First, the appropriateness of“Risk for Infection”was examined for the real nursing problem of 83 cases. The 66 cases of those were not appropriate because there were one or two risk factors only. Second, these 17 cases were further analyzed to identify the appropriateness of nursing outcomes and nursing intervention using the tool of analysis which was made by Nursing Outcomes Classification (NOC) and Nursing Interventions Classification (NIC). As a result, the nursing outcomes were not described about positive change of patients phenomena concretely. On the other hand, the nursing interventions were described related to the infection protection, the incision site care, the wound care, and the infection control concretely. Further when nursing outcomes and nursing interventions were thought for particular nursing diagnosis, the model which case-specific NOC and NIC were organized to particular nursing diagnosis would be supposed.
