急性期病棟における看護師の糖尿病ケアに対する認識と実施状況を明らかにすることを目的とした.糖尿病患者が入院する急性期病棟の看護師に無記名の質問紙調査を実施し,89部を分析した.急性期病棟における看護師は,糖尿病患者と関わることに対して,「時間がかかる」79.7%,「難しい」78.6%,「苦手である」56.8%とネガティブな思いをもち,「楽しい」,「やりがいがある」,「苛立つ」とはあまり思っていなかった.また,低血糖に関連する情報提供の実施状況は46.1〜51.7%であり,「患者の食事療法の摂取カロリーと塩分の量を伝える」など生活の中で実施する療養行動に関する情報提供の実施状況が低かった.糖尿病ケアの実施状況を高めるためには,患者の行動だけでなく患者の考え方や態度の変容にも視点を向けられるような支援や,退院後の患者の生活状況を病棟看護師に伝達できるシステム,糖尿病ケア以外のケアを実施しながらの糖尿病に関する情報提供が必要である.
The purpose of this study was to determine the awareness of diabetes care among nurses and status of providing to diabetes care in an acute care ward. An anonymous questionnaire survey was conducted with nurses in an acute care ward with diabetes inpatients, and 89 responses were analyzed. Nurses in an acute care ward had negative feelings about providing diabetes care to diabetics patients:, ”take a long time” 79.7%, ”difficult” 78.6%, and ”not like” 56.8%. Also they didn't think it was fun, rewarding, or frustrating. In addition, information provision related to hypoglycemia ranged from 46.1 to 51.7%, and the state of information provision on self-care implemented in daily life, such as ”explain the calorie intake and salt content of the patient's diet”, was low. In order to improve the implementation status of diabetes care, it is necessary to assistance that focuses not only on patient behavior but also on changes in patient thinking and attitudes, a system that can communicate the life situation of patients after discharge to a ward nurse and providing information on diabetes while taking care other than diabetes care.
