テント下病変による高次脳機能障害に大脳白質病変と微小脳出血が及ぼす影響

Effects of cerebral white matter lesions and cerebral microbleeds on cognitive dysfunction due to infratentorial lesions

要旨：テント下病変を有する脳血管障害患者の高次脳機能障害と大脳白質病変（White Matter Lesion；以下，WML），微小脳出血（Cerebral Microbleeds；以下，CMBS）との関連を明らかにすることを目的として，回復期リハビリテーション病棟に入院した患者の診療録を用いてケースコントロールデザインで検討した．その結果，脳室周囲病変の程度と前頭葉機能に関連する検査結果の間，および深部白質病変の程度と前頭葉機能に関連する検査結果の間に関連が認められた．また，infratentorialにおけるCMBSと前頭葉機能との関連が認められた．以上のことから，テント下病変を有する脳血管障害患者ではWML，infratentorialにおけるCMBSと前頭葉機能に関連があることが示唆された．

　To clarify the association between cognitive dysfunction and cerebral white matter lesions and cerebral microbleeds hemorrhage in patients with cerebrovascular disease with infratentorial lesions, patients' medical records of patients admitted to a convalescent rehabilitation ward in a case-control design were used. Associations were revealed between the degree of periventricular lesions and frontal lobe function, and between the degree of deep white matter lesions and frontal lobe function. An association was also found between infratentorial cerebral microbleeds. These findings suggest that cerebral white matter lesions, infratentorial microbleeds, and frontal lobe function are associated with cerebral white matter lesions and infratentorial microbleeds in patients with cerebrovascular disease with infratentorial lesions.
