前腕回内・回外障害に対する即席前腕回旋装具の試み

Dynamic quick-made forearm rotation orthosis for forearm pro-supination contracture

要旨：前腕部の骨折では前腕回内・回外障害をきたすことがある．これらの拘縮改善にはいくつかの前腕回旋装具が開発されているが，回内・回外を制御する装具は作製が困難で，現在でも矯正位で固定するスタティックなタイプが多い．そこで我々は可撓性プラスチックキャストを用いて，即席前腕回旋装具を考案した．本装具は作業療法士によって短時間に作製でき，動的機能を持つなどの特徴を有した治療用仮装具である．外傷性前腕回旋障害例に対して本装具を使用した結果，従来の装具と類似ないし，より優れた有用性が示唆された．今回は即席前腕回旋装具の紹介とともに，本装具の作製法について報告する．

　We often encounter forearm pronation-supination contracture due to forearm injuries such as bone fractures. There are some forearm rotation orthoses which correct pro-supination contracture. In general, these rotation orthoses are difficult to make, and the majority of these orthoses are static in function, and the efficiency of the correction is often limited.

　Therefore, we devised a dynamic quick-made forearm rotation orthosis by using a flexible plastic cast. This orthosis can be made by the occupational therapist in a short period of time.

　We applied this orthosis to three patients with limited forearm rotation secondary to various injuries. The results of using this orthosis were satisfactory, suggesting similar or better usefulness compared to conventional forearm rotation orthoses.
