精神病院における社会復帰センター3年半の取り組み—社会参加意欲に焦点を当てたOTプログラム

An occupational therapy program : reestablishing the drive for rehabilitation

要旨：精神病院の長期入院患者の社会復帰プログラムの一つとして，患者の「意欲」そのものに注目したOTプログラムを試みた．これは「社会復帰センター」という名称ではあるが，直接社会復帰を目指すものではない．退院・社会復帰に対して希望を失っている患者・スタッフが希望を見出し，意欲を引き出そうとするプログラムである．具体的には作業能力の向上よりも，むしろ意欲を引き出す治療的環境作りの工夫，すなわちグループワークの重視とフィードバックシステムとしての多職種の連携という工夫に重点が置かれた．こうして実際に退院していく患者も現われ，試みたOTプログラムの工夫がある程度有効であることが確認できた．

　It is important and difficult to rehabilitate chronic inpatients in mental hospitals. The difficulty of rehabilitation has often made all those involved pessimistic. For the purpose of reestablishing the drive for rehabilitation, attention was focused on a plan which involves occupational therapy. The plan includes group psychotherapy and a "feedback system" in cooperation with various therapists.

　The plan induced patients' drive for entrance into the community, and was useful for rehabilitation. Consequently, this issue encouraged inpatients and therapists alike.
