要旨：掌側ロッキングプレートの普及により橈骨遠位端骨折の手術成績は飛躍的に向上した．強固な固定により早期リハビリテーションが可能となり，良好な成績が獲得されるようになったが，三角線維軟骨複合体（triangular fibrocartilage complex；以下，TFCC）損傷が術後後遺症としてしばしば問題となる．TFCC損傷の中でも尺骨小窩断裂は手関節尺側部痛と遠位橈尺関節の不安定性を生じるため，当科では橈骨遠位端骨折に合併する同断裂に対し鏡視下縫合術を行い可及的早期にリハビリテーションを行っている．今回，少数ではあるが自験例を中心に後療法の工夫，術後成績を詳述した．良好な機能改善の獲得に加え，高い患者満足度が得られた．

　The treatment results of distal radius fractures (DRF) have improved dramatically by the spread of the volar locking plate. Early rehabilitation following rigid fixation has led to positive results, although triangular fibrocartilage complex (TFCC) injuries sometimes occur as a sequela. Foveal tear of TFCC injuries result in ulnar side wrist pain and instability of distal radioulnar joint. Therefore, we are doing arthroscopic repair on this tear-associated DRF, and are doing as much early rehabilitation as possible. In this study, we detail the rehabilitation efforts and postoperative results around a few of our cases. In addition to good functional improvement, patient satisfaction was high.
