要旨：術後の慢性疼痛は回復を遅らせ，患者満足度を低下させる．上肢運動器疾患における術後の痛みは，心理因子が重要となる可能性がある．そこで本研究は，上肢運動器疾患における痛みの強さの予測に心理因子が寄与しているか検討した．上肢運動器疾患に対して手術が施行された52名を対象に，リハビリテーション開始時に不安感，抑うつ，破局的思考を測定し，さらに術後8週に痛みの強さを測定した．これらを重回帰分析にて検討した結果，痛みの予測因子として破局的思考が抽出された．このことから，心理因子は上肢運動器疾患において痛みの強さの予測因子であることが示唆された．

　Postoperative chronic pain is associated with delayed recovery wand reduced patient satisfaction. Psychological factors may be an important determinant of postoperative pain in patients with upper limb musculoskeletal disorders. The aim of this study was to examine the contribution of psychological factors as a predictor of pain intensity in upper limb musculoskeletal disorders. Fifty-two post-operative upper limb patients were measured for anxiety, depression and catastrophizing upon initiating rehabilitation. In addition, pain intensity was measured eight weeks after surgery. Using multivariate analyses, catastrophizing was the main predictor of pain intensity. These data suggest that psychological factors predict postoperative pain intensity in patients with upper limb musculoskeletal disorders.
