言語聴覚士と介護支援専門員の連携について

Cooperation between speech-language-hearing therapists and care managers

　介護保険制度の導入から5年が経過し，それまでの制度運営の状況を踏まえて立案された改正介護保険法が平成18年4月に施行された．医療保険診療報酬との同時改定となり，医療，介護に携わる専門職にとって大きな変化がもたらされることとなった．このような社会情勢において，高齢者の在宅支援にあたって言語聴覚士(以下ST)にどのような働きが期待されるかを，STを基礎資格とする介護支援専門員(以下ケアマネ)の立場から検討した．まずはじめに，STがケアマネの業務を行って効果的であった2症例を紹介した．続いて，福井県敦賀市にある居宅介護支援事業所のケアマネに対しSTとの連携について問うアンケートを行い，①STという職種に対する理解が十分ではない，②介護保険制度を有効に活用することでSTの提供するサービスによって機能改善，あるいは生活の質の向上が見込まれる対象者が存在する，という結果を得たことを報告した．今後，サービス担当者会議を中心としたSTとケアマネとの情報交換の機会を適切に設定していくことが重要であると提言した．

　In April 2006, the revised long-term care insurance law, which includes the changes deemed necessary after the first five years of the system, was put into effect.　Revised at the same time as medical fees, this revision of the law has resulted in major changes for professionals involved in medical and long-term care.　We therefore examined the roles expected of speech-language-hearing therapists (ST) in home care for the elderly amidst these conditions from the perspective of a care manager (CM) with the basic qualifications of an ST.　We first present two cases in which an ST effectively performed a CM's tasks.　Next, we report the results of a survey conducted on the cooperation between ST and CM of home care providers in Tsuruga City, Fukui Prefecture.　We found that:①CM's understanding of the profession of speech-language-hearing therapy is inadequate and②there are individuals with the potential for functional improvement or improved quality of life (QOL) as a result of service provided by ST thanks to the effective use of the long-term care insurance system.　We suggest that securing adequate opportunities for information exchange between ST and CM, such as care conferences, should be given higher priority in the future.
