クリップレス内視鏡下手術を目的とした結紮補助鉗子“Endo-Deschamps”の開発とその使用経験

New slender steel needle with a long handle and a slit in its curved end, used for helping ligation during endoscopic surgery

　18世紀のフランスの外科医Deschampsが考案したligature needleを改良して内視鏡下手術で使用可能な“Endo-Deschamps”を試作した．全長は43cm，ステンレス性，10mmのポートから挿入可能で，先端は10mmにわたり半円状にカーブ（R−4）し，倒し角度は105度である．最先端には幅0.2mm，深さ2mmのスリットが形成されており，太さ3-0までの糸が掛けられる．本鉗子の試験使用において胆嚢管や主要動・静脈の結紮が比較的容易に可能であった．結紮や縫合は手術の基本操作であり，内視鏡外科においても必須の手技である．本鉗子は今後クリップレス手術を検討していくうえで有用な機器と考えられた．

The ligature needle, made by Dr. Deschamps, French surgeon in 18 th century, has been widely used in the Japanese surgical field. We remade the Deschamps' needle so that it could be used in laparoscopic surgery. This new device, named “Endo-Deschamps” is a slender steel needle with a long handle, 43 cm in length and a slit in its curved (R4) end. It is possible to insert it through a 10-mm port and to hook the 3-0 suture onto the end slit, which is 0.2 mm in width and 2 mm in depth.
