LigaSureTM Systemを用いた新しい胸腔鏡下肺部分切除術—Thoracoscopic “Sutureless and Stapleless” Surgeryの治療成績

Novel Surgical Techniques Using LigaSurenTM in Thoracoscopic Pulmonary Resection

　自動縫合器（stapler）を用いた胸腔鏡下肺部分切除術は現在，呼吸器外科領域の様々な疾患に対して広く普及している．しかし，嚢胞性肺疾患においては特にその使用頻度の高さが医療経済の面から問題となっており，呼吸機能上の問題点も指摘されている．これらの問題点を解決するために，われわれは新しいバイポーラシステムであるLiga Sure^TM systemの肺部分切除術における有用性について検討した．肺組織におけるLigaSure^TMのシール強度に関しては，よりいっそうの改良が必要であるが，短期の手術成績は従来法に比べて遜色のないものであった．本法は今後，staplerに依存しない新しい胸腔鏡下手術へと昇華することが期待される．

Thoracoscopic pulmonary resection using endostaplers has been widely used in different diseases of general thoracic surgery. However, especially in pulmonary cysts, both the loss of pulmonary function due to the increase of normal lung tissue resected and the economical efficiency have become problematic as the use of endostaplers increased. To overcome these problems, we employed a new sealing technique using Liga Sure^TMVessel Sealing System (Tyco Healthcare, Valleylab) which was a new vascular sealing bipolar technology authorized by FDA in the USA.
