リンパ節郭清を伴った腹腔鏡下膵体尾部脾臓切除術を施行した膵体尾部癌の1例

Case Report: Laparoscopic Distal Pancreatectomy with Extended Local (D1+a) Lymph Node Dissection for Pancreatic Carcinoma

　腹腔鏡下手術は膵臓外科の領域にも拡がり，いくつかの施設から腹腔鏡下膵体尾部切除術の報告がなされている．しかし，膵液瘻や郭清の問題があり，適応は一部の良性疾患に限られているのが現状である．症例は76歳の女性で，食思不振と糖尿病の増悪のため，当院に入院した．検査で膵体尾部癌と判明し，D_1＋a郭清を伴った膵体尾部脾臓切除術を施行した．この術式は，超音波メスを用いた膵切離手技と，超音波凝固切開装置を用いた腹腔動脈周囲および上腸間膜動静脈領域の安全・確実な郭清手技により可能となった．術後経過は良好で，第3病日から経口摂取を開始し，早期退院が可能であった．腹腔鏡下膵体尾部切除術は膵臓癌の有用な治療法になると思われた．

Laparoscopic surgery for pancreatic diseases has been gaining acceptance recently. Several reports of laparoscopic distal pancreatectomy have appeared in the literature. Its indication is still confined to benign dis-orders since there are not enough data to indicate whether this procedure is potentially curative and can be safely performed for malignant tumors (e.g. number of dissected lymph nodes, major complication rate such as pancreatic fistula). A 76 y/o female was admitted to our hospital for anorexia and exacerbation of diabetes.
