Training System in Orthopedic Endoscopic Surgery

　脊椎外科で本格的に内視鏡下手術が行われるようになったのは1995年からであるが，1997年には第1回の脊椎手術手技講習会が開催された．以後，毎年2回開催され，これまでに9回が開催されている．整形外科だけではなく，脳神経外科の先生方の参加も多く，これまでの参加者の延べ人数は400人に達する，また，手術手技の実技指導には内視鏡外科医に依頼し，他科の協力が必要である．内視鏡下手術トレーニングセンターを使用してブタを用いた手術手技講習を行うが，脊椎における内視鏡下手術は大別すると（1）胸腔鏡，（2）腹腔鏡，（3）後腹膜腔鏡，（4）脊椎後方からの内視鏡，の4部門に分けられる．技術習得には各人の研鑚が必要だが，講習会を通じての意見交換も重要である．臨床実施にあたっては日本内視鏡外科学会のガイドラインに沿った形で内視鏡下手術を行うべく指導してきた．一定レベル以上の技術を保つためには，われわれ独自の難易度の設定や上級者のトレーニング・システムの構築も必要である．

Endoscopic spine surgery in our country was started in 1995. The first endoscopic spine surgery seminar was held in 1997. after that. the seminar was held twice a year. Over 400 participants experienced the endoscopic spine surgery in animal laboratory. We orthopaedists need to cooperation of endoscopic surgeons who are able to teach us how to use the new instruments and how to approach to the spine. Attendants learn four kind of ap-proaches; 1) thoracoscopy, 2) laparoscopy, 3) retroperitoneoscopy, 4) posterior approach to the spine using endoscope.
