低分子ヘパリンによる周術期凝血能の変化とその安全性に関する検討—周術期肺塞栓症予防の可能性

Discussion on Perioperative Changes of Coagulation Ability due to Administration of Low Molecular Weight Heparin and Its Safety: Expectation on Prevention of Perioperative Pulmonary Embolism

　今回，腹腔鏡下手術における周術期肺塞栓症（以下，PEの予防を目的として深部静脈血栓症（以下，DVT）のハイリスクと予測される14症例に対し，その予防法として低分子ヘパリン（lowmolecular weight heparin：以下，LMWH）の術前1回静注法を試み，その安全性とその予防の可能性につき検討した．その結果，術後の出血は少量で，凝血学的検査値は正常範囲内で推移し，また血中抗Xa因子活性は執刀開始後約2時間まで至適濃度を維持することが可能であった．また，明らかなDVTおよびPEの発症を認めなかった，以上の結果から，LMWHは安全に使用が可能であった．今後，ハイリスク症例に対するLMWHの周術期DVT／PE予防法への適応拡大と皮下注製剤の供給が必須と思われた．

Safety of a preoperative intravenous bolus of low molecular weight heparin (LMWH) was evaluated to pre-vent perioperative pulmonary embolism (PE) during laparoscopic surgery in 14 patients who were considered to be in high risk group for deep vein thrombosis (DVT). The possibility for prevention of the disease has been discussed. This method resulted in a small amount of postoperative bleeding. The hematological data showed that the coagulation test results remained within normal limit and it was indicated that the serum anti-Xa factor activity could be maintained at optimal level for 2 hrs after beginning the operation.
